Graduates of our chaplaincy program are eligible to apply for licensure. If you have successfully completed our chaplaincy program, and would like to apply for licensure, please complete the brief form at right. 

NOTE: There are supporting documents which are required, and which you will be prompted to upload. Please gather the following PRIOR to beginning the form at right. You will need:

• a copy of your resume (in Microsoft Word or PDF format), and
• a copy of your Chaplaincy Program Completion Certificate (in Microsoft Word or PDF format).
